Why EBV Suddenly Shows Up

I don't think a week goes by where I don't have at least one patient asking me about Epstein Bar virus or where it doesn't show up on some lab work. The past couple weeks, though, I have to kind of shake my head and wonder: is Epstein Barr making a comeback? Why am I seeing so much more reactivation than I ever have before? Twenty years ago, we rarely talked about it outside of mono. Today I'm seeing patients with fatigue, brain fog, joint pain, swollen lymph nodes, chronic infections, histamine intolerance, autoimmune disease, and guess what keeps showing up? Evidence that their immune system is struggling to keep Epstein Barr under control. So why? Is EBV actually becoming more active, or are we finally asking the right questions?

EBV Basics And Lifelong Latency

If you're wondering what Epstein Barr virus is, it is one of the most common viruses in the world. It belongs to the herpes virus family, specifically human herpes virus 4 or HHV4. And once you become infected, it remains in your body for life. About 90 to 95% of adults worldwide have been affected with EBV, often during childhood or adolescence. Many people never realize they had it because the initial infection may cause mild symptoms or none at all. So when you get EBV, usually in children, it causes little or no illness. In teenagers and young adults, it can cause infectious mononucleosis, also known as mono or the kissing disease. Common symptoms of acute EBV usually include extreme fatigue, sore throat, fever, swollen lymph nodes, enlarged tonsils, headache, body aches, and in some cases, you can get an enlarged spleen. That's what made it dangerous. Most people recover over several weeks, although fatigue can last for months. After the initial infection, EBV enters this latent or dormant state. It hides primarily inside B lymphocytes. It's a type of white blood cell that's involved in the immune system. A healthy immune system usually keeps a virus under control so it remains inactive and doesn't cause any symptoms. This isn't about getting EBV over and over. It's about losing control over a virus you already have.

Reactivation Symptoms And The Perfect Storm

So what is reactivation? So the immune system normally keeps the EBV dormant. When the immune system becomes overwhelmed, the virus wakes back up. Not necessarily causing mono, but it causes chronic inflammation. And symptoms you can have can include fatigue, brain fog, increased anxiety, muscle aches, joint pain, histamine issues, swollen glands, poor exercise recovery, sleep disturbance, even hair loss. But the biggest I see is autoimmune flares. This is probably one of the most interesting things when we talk about reactivation. Usually people have what I call the perfect storm. The first one always starts with chronic stress because there's cortisol dysregulation, there's sympathetic dominance, and you're not sleeping very well. Then, you know, the C word, COVID. COVID doesn't cause EBV, but severe viral illness can disrupt your immune surveillance. So several studies have shown EBV reactivation can occur during or after a COVID infection in some individuals. Poor nutrition is the next because of the sad diet or just because of how depleted everybody is in their minerals. Zinc, selenium, vitamin D, iron, magnesium, they're all necessary for immune regulation. Then gut dysfunction. 70% of your immune system, it lives in your gut. So if you have leaky gut or dysbiosis, if you have food sensitivities, chronic inflammation, the immune system becomes very distracted. Mold, that is a huge immune stressor. Toxins, heavy metals, environmental chemicals, even hormonal changes. This is perimenopause, menopause, postpartum, or even if you just have low testosterone, blood sugar. Um, I talk about this at all of my appointments between insulin resistance, just having high glucose, or having that inflammation.

Autoimmunity Risk And Immune Dysregulation

People ask me if EBV causes autoimmune disease. And I don't think EBV causes autoimmune disease by itself. Um, research has linked EBV with increased risk of conditions like lupus, MS, Hashimoto's, or rheumatoid arthritis. But one thing I want to be really careful about is creating fear around this Epstein Barr virus. If I tested everybody listening today, the majority would have positive antibodies. So the question I ask isn't, do you have EBV? The question is, why is your immune system no longer keeping it under control? That's a very different conversation. I like to think of your immune system like the conductor of an orchestra. When every section is playing together, everything stays balanced. Viruses like EBV are present, but they're quiet. They're essentially sleeping. Now imagine the conductor becomes overwhelmed. Maybe you're under chronic stress, you're sleeping just four or five hours a night, your blood sugar is out of control, you have nutrient deficiencies, your gut is inflamed, maybe you're going through menopause. Um, maybe you've had multiple infections over the past few years. None of those things may seem connected, but together they can throw your immune system off balance. That's why I call it immune dysregulation. Immune dysregulation doesn't necessarily mean that your immune system is weak. In fact, many people with autoimmune conditions have an immune system that's incredibly active, but it's active in the wrong ways. It's overreacting to some things while losing control over others. Instead of staying focused on keeping latent viruses like EBV in check, the immune system becomes distracted by chronic inflammation, ongoing stress signals, poor nutrition, or other immune challenges. So when someone asks me, is EBV causing my symptoms? My answer is usually maybe, but I want to know why it's active in the first place. Because if all we do is chase a virus and never address the reason your immune system became dysregulated, we're really missing the bigger picture. And I know I ask a lot of questions. Why is your immune system overwhelmed? What is driving your inflammation? Is it your gut? Is it your hormones? Is it insulin resistance? Is it nutrient deficiencies? Is it chronic stress? Are your poor sleep? Are you exposed to mold or other environmental exposures? When we improve the terrain your immune system is working in, we often see people dramatically get better. Not because we magically erased EBV, but because we helped restore balance. And that's the goal. Not to live in fear of a virus that most of the population carries, but to build a resilient immune system that can do what it was designed to do. Think of EBV like weeds in a lawn. You keep pulling the weeds over and over, but the soil is unhealthy. They're gonna keep coming back. Functional medicine focuses on improving the soil so that the weeds stop taking over.

Mast Cell Activation And Histamine Issues

Another question I get is about mast cell activation. Um, mast cell activation, also known as MCAS, is a condition in which mast cells become overly reactive and release inflammatory chemicals inappropriately or excessively. Mast cells are immune cells that live throughout your body, especially in your skin, your lungs, your gut, your sinuses, um, blood vessels, and your nervous system. So it's pretty much your body's first responders. When they detect a threat such as an infection, injury, or allergen, they release dozens of chemical messengers, including histamine, tryptase, prostaglandins, leucotrienes, and cytokines. These chemicals are there to protect you. But if mast cells become dysregulated, they can release these mediators too easily or too often. And that causes a lot of symptoms throughout your body. And because mast cells are found almost everywhere, symptoms can infect multiple organ systems. Um, most common I hear about are flushing, hives, itching, facial swelling, sinus congestion, heart palpitations, dizziness, lightheadedness, anxiety, or even panic sensation, headaches, brain fog, abdominal pain, diarrhea, bloating, nausea, shortness of breath, and even sensitivity to foods, smells, heat, medications, or alcohol. I hear a lot of people say, I am suddenly reacting to everything. And although current research has not proven that EBV directly causes MCAS, but there are biologically plausible reasons why EBV may contribute to mast cell activation in some people. When Epstein-bar virus infection reactivates, it does stimulate the immune system and increases inflammatory signaling. So in many people, this chronic immune activation may make mast cells more likely to activate and release histamine and other inflammatory mediators. So first we have EBV, and that causes chronic immune activation, which then causes immune dysregulation, which then we have increased mast cell reactivity, which then causes histamine-related symptoms. So this doesn't happen with everybody that has EBV, which is why genetics, gut health, hormones, environmental exposures, and other factors, you know, they all matter. At Harmony Hub Health, I wouldn't stop at saying, hey, I think you have MCAS. What I ask is, why are your mast cells so active? Is EBV contributing? Is your gut inflamed? Are you nutrient deficient? And is mold exposure keeping your immune system activated? Are your hormones changing? Is chronic stress keeping your nervous system in overdrive? EBV may be one piece of the puzzle, but it's rarely the entire puzzle. The goal is to identify and address the factors driving immune dysregulation so the immune system can regain balance and mast cells become less reactive over time.

How To Read EBV Antibody Panels

Now let's talk about testing for EBV. One of the biggest mistakes I see is somebody got some EBV panels done by their doctor and they see a positive antibody and they immediately think, ah, I have an active Epstein bar infection. Well, not so fast. Antibodies tell us what your immune system has seen and how it's responding. They don't always tell us what the virus is doing today. That's why we have to interpret them alongside your symptoms and the rest of your health picture. Think of EBV, VCA, IGM. This is the new alarm. Um, this is the fire alarm. It's usually the first antibody your body makes when you're infected with EBV for the first time. It generally appears early and fades over weeks to a few months. If someone has a positive VCA IgM, it does raise the possibility of a recent or acute infection. Um, sometimes IgM can reappear with the activation or be falsely positive. So it isn't interpreted in isolation. The EBV, VCA, IgG, this is your immune system's memory. Once you've been infected, VCA IgG usually stays positive for life. That's completely normal. In fact, most adults have a positive VCA IgG because most of us have been exposed to EBV. A positive VCA IgG by itself does not mean that the virus is active. And this is where people get confused. Sometimes I'll see a VCA IgG that's not just positive, it's extremely elevated. And a very high IgG doesn't automatically mean the virus is actively replicating. It may reflect a robust immune memory, and some people maintain high antibodies for years. However, when I see very high IgG together with symptoms like profound fatigue, swollen lymph nodes, brain fog, or joint pain, it does prompt me to look more closely for evidence of reactivation and more importantly, reasons why the immune system is under stress. Um then we have EBV EAD. This is early antigen. And this is one of the more interesting markers because unlike the VCA IgG, which usually stays positive for life, the early antigen antibodies are often absent after recovery. So if early antigen becomes elevated, especially in someone who has symptoms, it can be a clue that the virus has become more active again. It's not a perfect test. Some healthy people can have persistent early antigen antibodies, and some people with reactivation won't, but in the right clinical context, it can add another piece to the puzzle. Um, another important antibody is the eBNA Ig or Epstein Bar nuclear antigen. This antibody typically develops a few months after the initial infection and usually remains positive for life as well. If your EBNA IgG is positive, it tells me you've had EBV in the past, but it doesn't tell me that the virus is active today. So I don't look at one antibody. I like to look at the pattern. Um, for example, here's what I had this past week: one positive VCA IgM with no eBNA IgG. So that can suggest a recent primary infection. I had another person positive VCA IgG and positive EBNA IgG with negative early antigens. So this is the pattern I see in most healthy adults who had EBV years ago. When someone has very high VCA IgG, a positive early antigens and symptoms that fit, well, then reactivation does become a consideration, but it's still only part of the overall assessment. I have to ask the most important question: why is this person's immune system struggling to keep EBV in a dormant state? And that's where functional medicine really comes in. At Harmony Hub Health, we don't chase antibodies, we investigate the person. Um, why is the immune system dysregulated? Okay. One thing I always remind patients is that the actual antibody number doesn't necessarily correlate with how sick someone feels. I've seen people with very high EBV IgG who feel perfectly fine. And I've seen others with more modest elevations who are very symptomatic. That's why lab interpretation should always be done in the context of symptoms history and the rest of the clinical evaluations and not by the numbers by themselves. This is a reason why we don't just stop at EBV antibodies. I also want to see a complete system history, comprehensive labs, how is your iron? How's your vitamin D, your zinc, your B vitamins, your thyroid, inflammation, fasting insulin is something that I think is non-negotiable to check. Um, even a GI map can be helpful, IgG food sensitivities, HTMA, cortisol patterns in hormones. Um, I don't believe the virus is usually the root problem. The virus is often telling us something about the terrain. If your immune system is healthy, EBV usually stays quiet. So I don't sit around and wonder how do I kill the virus? I want to figure out why your immune system lost control.

Treat The Terrain Not The Virus

There is no medication that eliminates EBV from the body. Okay, management focuses on supporting recovery and addressing factors that may contribute to immune dysfunction. So the things that we really work on are adequate sleep and stress management, good nutrition and correction of any nutrient deficiencies, um, treatment of any other medical conditions, um, gut health, hormone imbalances, or metabolic issues when appropriate.

Ozone Major Autohemotherapy Explained

My go-to therapy at Harmony Hub Health is major autohemotherapy or MAH. This is ozone. And I know it sounds intimidating, but it's actually a fairly straightforward procedure. I draw a small amount of your blood into a sterile closed system. Um I use medical-grade ozone, a carefully controlled mixture of oxygen and ozone, and I introduce that to your blood. The ozone reacts almost immediately with components in the blood, creating signaling molecules called ozonites. The blood is then gently given back to your body. Um and the ozone itself doesn't circulate throughout your body, it reacts within seconds. What your body responds to are the biological signals that reaction creates. I'm not using ozone because I believe it's directly killing the Epstein bar virus inside your body. Remember what we talked about earlier. Almost everyone carries EBV. The issue isn't usually the virus itself, it's the immune system that has become dysregulated and isn't keeping it dormant. The goal of ozone therapy is to support the body's own regulatory systems. And there's a lot of ways that ozone therapy can help. The first is that immune modulation. And one of the most interesting effects of ozone is that it appears to help modulate the immune response. Think of your immune system again as that orchestra. Sometimes it's underactive, sometimes it's overactive, sometimes it's simply out of sync. Rather than simply boosting the immune system, which isn't always desirable, especially in autoimmune conditions, we're trying to help it become more balanced and better regulated. Benefit number two is improved oxygen delivery. Ozone therapy has been shown to increase levels of 2-3 DPG. This is 2-3 diphosphoglycerate within the red blood cells. That helps hemoglobin release oxygen more efficiently to tissues. Many patients with chronic fatigue don't necessarily have a problem carrying oxygen. They may have difficulty delivering oxygen where it's needed the most. Better oxygen delivery may support healing and cellular function. Um, benefit number three is hormesis. This is training the body's defenses. My favorite way to explain ozone is that it's like sending your body to the gym. A workout creates a controlled stress. That stress is not harmful. It's what stimulates your muscles to become stronger. Ozone works in a similar way, it creates a very small controlled oxidative challenge. Your body responds by activating its own antioxidant defense systems, including enzymes like superoxide, dismaltase, um, catalase, and glutathione peroxidase. Instead of supplying antioxidants from the outside, we're encouraging your body to increase its own natural defenses. Um, benefit number four is reduced chronic inflammatory signaling. When the immune system is chronically activated, inflammatory signaling can stay elevated. Ozone by itself is not the perfect treatment. Um, if someone has EBV reactivation because they're only sleeping four hours a night, they have insulin resistance, low vitamin D, no zinc, gut inflammation, and high stress, no amount of ozone is going to fix all of that. Minerals HTMA And Food Sensitivities

virus. My favorite is combining the ozone therapy with the functional medicine testing. Two of the biggest pieces of the puzzle usually are mineral balance and hidden food sensitivities. Your immune system does not work without minerals. Every immune cell relies on minerals to communicate, produce energy, fight infection, and regulate inflammation. For example, zinc is critical for antiviral immunity and normal T cell function. Magnesium is involved in hundreds of enzymatic reactions, including energy production and regulation of inflammation. Um, selenium supports antioxidant enzymes like glutathione peroxidase, helping to protect cells from oxidative stress. Um, copper and iron are also essential, but they need to be in balance. Too little or too much can really affect your immune system. When these minerals are out of balance, the immune system may not respond as efficiently as it should. And that's one reason why I use the HTMA or hair tissue mineral analysis. Instead of only looking at blood levels, HTMA can provide another perspective on long term mineral patterns and how the body may be handling stress. We use that information to personalize nutritional support, not just to replace deficiencies, but to help restore balance. The second piece is the food sensitivities. And I'm not talking about food allergies like IgE. Eat that cause immediate hives or anaphylaxis. I'm talking about delayed immune reactions that in some people may contribute to ongoing inflammation. When the immune system is constantly reacting to foods that a person isn't tolerating well, it's spending energy managing that inflammation. If someone has EBV reactivation and we're also seeing significant food sensitivities, reducing those triggers may help lower the overall inflammatory burden on the immune system. The goal is not to avoid those foods forever, it's to calm the immune system, support gut healing, and stop that leaking gut. And then when appropriate, we work towards expanding that diet again. Okay. This is why we don't treat EBV in isolation. We combine comprehensive lab testing, mineral balancing, gut restoration, hormone optimization, blood sugar regulation, stress and sleep support. Sometimes we have to incorporate that flow pod, um, targeted supplementation, and for many patients, the ozone therapy. When you improve the terrain your immune system is working in, you're giving it the best opportunity to keep latent viruses like EBV under control. Um, we're not trying to suppress a virus. And I'm not even saying that ozone is going to eradicate EBV. We're helping the immune system become resilient so that it can do its job and do what it was designed to

Who Should Get Evaluated

do. So who should think about getting evaluated? If you have fatigue that never fully resolves, if you're suffering with brain fog, recurrent illness, autoimmune disease, histamine intolerance, chronic swollen glands, hair loss, um, that's how I found one of these just this past week. Um, persistent inflammation, or symptoms that have been dismissed as normal, you might want to consider getting evaluated. I always remind my patients that a positive EBV antibody doesn't automatically mean the virus is causing all of your symptoms, but that's why we never diagnose based on one lab alone. We look at the whole picture: your history, your symptoms, your immune health, your hormones, your gut function, nutrient status, and lifestyle. If EBV is part of the story, our goal isn't simply to fight the virus, it's to build an internal environment where your immune system can do what it's designed to do. If there is one thing I hope you take away from today's episode, is this EBV is not the enemy. In fact, almost all of us carry it. The real question is not how do I get rid of Epstein Barr? The better question is, why is my immune system struggling to keep it quiet? Your body is incredibly intelligent. When viruses reactivate, inflammation persists, or symptoms keep coming back, it's often a sign that your body is asking for help, not that it's failing.
